 Test tube babies are babies that are conceived outside the woman’s body. A baby conceived through IVF is commonly known as a test tube baby. In-Vitro Fertilization (IVF) is the type of fertilization where the egg is combined with the sperm outside the human body. The process involves monitoring and stimulation of egg production with hormone therapy, removing the eggs from the ovary and combining the sperm with ovum and letting it fertilize them in a liquid in a laboratory. The fertilized egg is cultured for 2-6 days and allowed to divide 2-4 times in the test tube. These eggs are moved to the uterus of the mother or surrogate hoping this would result in a successful pregnancy.

Test tube baby was first delivered in the year 1978 with the efforts of scientist Robert Edwards and gynecologist Patrick Steptoe.    The IVF procedure includes the following steps:

  • Suppressing the natural menstrual cycle and egg stimulation   The patient is given medication usually in the form of injection for about 2 weeks to suppress the natural menstrual cycle. Fertility drugs containing FSH (Follicle stimulating hormone) is given to the patient. The FSH hormone induces production of more eggs than usual. Vaginal ultrasounds are used to monitor the ovaries.  
  • Retrieving the eggs   The retrieval of eggs is done through a minor surgical procedure. This involves inserting a hollow needle through the vagina and into an ovary. Using a suction device the eggs are retrieved one by one. Research suggests that collecting 15 eggs in one cycle gives the highest chance of successful pregnancy. Test tube babies can be made using frozen or donated eggs.
  • Insemination and fertilization   The male sperm sample is collected and mixed with the eggs in a controlled environment i.e. a Petri dish for insemination. The eggs and sperm are combined for few hours to allow fertilization and production of embryos. Fertilized eggs are considered as embryos. If the chances of fertilization are low, ICSI can be considered where the sperm is directly injected into the egg. One or two of the best embryos are selected for transfer into the uterus. Hormones like progesterone or human chorionic gonadotrophin (hCG) are given to the woman to help the lining of the womb which is about to receive the embryo.
  • Embryo transfer Embryo transfer is an important step in the process of IVF or test tube baby method. Based on the assessment the doctor suggests the patient whether they should opt for single or multiple embryo transfers. The transfer of a single high-quality embryo decreases the risk of twins or triplets. But, when no ideal embryos are available, the doctor may transfer more than one embryo to increase the chance of pregnancy. Anesthesia is not required for this process.

 In this step, the embryos are transferred into the woman’s uterus with the intent of establishing a successful pregnancy. During the transfer, the doctor inserts a soft catheter through the cervical canal and is advanced into the uterine cavity. After insertion, the contents of the catheter are expelled and the selected embryos are deposited. When the embryo sticks to the lining of the womb, healthy growth of the embryo can begin.  Ultrasound is used to monitor the entire process. Ultrasound helps to determine the optimal target point for embryo placement. The patient is advised to rest for 5-6 hours after the process. A pregnancy test is conducted to find out the exact situation.
